I'm a new patient. What time should I arrive and what should I bring to my appointment? Please arrive 15 minutes early for your appointment to fill out the necessary paperwork: [ Patient Demographic Form | Patient Medical History Record ]. Please bring your driver’s license or picture identification, insurance card(s) and a list of current medications. If you have copies of recent lab work or notes from a recent office visit with your PCP, you may bring those as well.
How do I schedule an appointment? Please call to schedule an appointment. Please verify with your insurance provider whether this visit will require a referral. You may also request that your PCP call to arrange the appointment.
What should I do if I leave a message for the nurse and I do not get a return call right away? We will always try to return all calls on the same day. However, there maybe mitigating circumstances (such as the volume of patients we are attending to) that may extend our response time. If you have not received a return call from our office in a reasonable amount of time, please call us again and inform the office staff that this is your second call.
Who should I call in the event of a medical emergency? Please call 911. The emergency response team is equipped to best respond to your initial needs in this situation. If you are unable to get to a phone, please proceed to the nearest emergency medical facility.
What are the hours of operation? Our office hours are from 8:00 AM to 5:00 PM, Monday through Friday. We are closed on the weekends and for most major holidays.
How do I get my medication refilled? Please leave a message for the nurse to let her know which medication needs to be refilled, along with your name, date of birth, and the pharmacy number. As long as you have had a recent office visit with a physician or midlevel, this will be called to the pharmacy the same business day.
Will I have to pay anything at the visit? Possibly. This depends on your insurance plan. You may check your insurance card under specialist or contact your insurance company prior to the visit. The office will verify insurance coverage and collect any copayment or deductible at check-in.
What if I have a question about my bill? Please contact our office at 770-534-9014 and ask to be directed to the billing department. However, many billing questions can be answered by reviewing your Explanation of Benefits (EOB) that you receive by mail from your insurance company.
What if I need to reschedule my appointment? Please call our office at 770-534-9014 to reschedule. We ask that you reschedule at least 24 hours in advance.
What forms of payment do you accept? We accept Cash, Check, Money Order, Cashier’s Check, Visa, MasterCard, and Discover. We currently do NOT accept American Express.
